The Remington 700 triggers had safety concerns for years before they finally addressed them. Then there's the Sig P320 which had issues with drop safety. And let's not forget about Taurus, whose quality control has been all over the place for ages.

Think about those early pistol optic plates. A lot of them would loosen up or shift zero after a while. The idea was good but some of the first designs were obviously rushed.
